Authorization for Repairs: Submitting this Service Request form serves as your acknowledgment of the following: 1) You authorize Simplified Yachting Services to proceed with the repair work requested; 2) You agree that Simplified Yachting Services is not responsible for a) delays caused by unavailable parts b) loss/damage to the vessel or to articles left in the vessel due to fire, theft, vandalism, or freezing; 3) You grant permission for Simplified Yachting Services employees to operate the vessel for the purpose of testing and/or inspection; 4) You have insurance covering the above-referenced vessel that you will maintain throughout the time that the vessel is being repaired by Simplified Yachting Services.
